He was predeceased by: his wife Rose Kozachuk (Pawluk); and his siblings, Mary Barbato, Paul, George, Walter and Gregory Kozachuk.
He is survived by: his children, Kevin Kozachuk (Maureen) of Woburn Dear and Karen Hunt (Kevin) of Woburn Dear. He is also survived by loving nieces and nephews.
