He was predeceased by: his parents, Roy E. Ellsworth, and Hazel Ellsworth, Sr. (Lowry); his wife Elaine Anne (Nassif); and his siblings, Elizabeth Patten, Shirley Ellsworth, Arlene Rondeau, Joyce Ellsworth, Helen Bailey, Ronald Ellsworth, Edward Ellsworth and Roy Ellsworth, Jr..
He is survived by: his daughters, Lori A. Carter of Middleboro and Nancy Ellsworth of Adams; his grandchildren, Seth Carter, Nathan Carter, Victoria Carter (Jarell Samuel), Jillian Green (Michael), Megan Adams and Morgan Adams; and his great grandchildren, Jake and Elle. He is also survived by nieces and nephews.
