He was predeceased by: his parent Frank S. Slowik; and his sibling-in-law Fernando Monteiro. He is survived by: his spouse Cheryl A. Slowik (Duckett); his parent Emily B. Slowik (Klecha); his stepson Brian Furtado (Kimberly); his siblings, Patricia "Pat" Monteiro of New Bedford, Stephen Slowik of New Bedford and Edwin Slowik; his brother-in-law Dino Mello; his grandchildren, Peyton and Bryson; and his sibling Janice Mello. He is also survived by several nieces and nephews.
