He was predeceased by: his parents, Leo and Charlotte Blake Sr. (Moore); and his sister Laura Blake Mondoux. He is survived by: his friends, John and Linda Berry; his cousins, John Blake, Cynthia Siermienski of West Springfield, Betty Warren of Westfield and William Blake of Hawaii; and his nephew Francois Mondoux.
