She was predeceased by: her father Edward T. Koziak Sr.. She is survived by: her mother Susan M. Koziak (Bielik) of Webster; her brother Edward T. Koziak Jr.; her wife Sherry of Charlton; and her sisters, Madeline Nawrocki (Peter) of Dudley and Jeanne Hackenson (David) of Dudley. She is also survived by several nieces, nephews, great nieces and nephews, and many friends.
