He was predeceased by: his great grandchild Adrian Lewis; and his parent William Cudak. He is survived by: his wife Thelma G. Cudak (Codding); his children, Robert V. Cudak (Lisa) of Gardner, Philip M. Cudak of Webster, Karl W. Cudak (Barbara) of Westminster and Lilia Arsenault (Mike) of Westminster; his grandchildren, Gina L. Cudak of Gardner, Nicole Bielicki of Franklin, Joshua Cudak of Westminster, Michael Cudak of Westminster and Karlene Cudak of Westminster; his great grandchildren, Anthony Lewis, Avery Lewis and Adrianna Justis; his half-brothers, Dennis Smith of Framingham, Edwin Smith of Medway and Walter Smith of Uxbridge; his parent Anelia Cudak (Kostra); and also Polina of Westminster. He is also survived by several nieces, nephews and cousins.
