He was loved and cherished by many people including: his parents, Ethel Meltzer (Krofschek) and Solomon Meltzer; his wife Joan Meltzer; his stepdaughter Jessica Sommerville (Joseph Stulpin); his children, Wendy Hurwitz (Barry), Elizabeth Bloom (Seth) and Jonathan Meltzer (Alissa); his grandchildren, Rachel Hurwitz, Joshua Hurwitz, Adam Hurwitz, Talia Bloom, Alex Bloom, Daniel Bloom, Jeremy Meltzer and Julia Meltzer; and his former wife Bonnie Rosenberg.

In lieu of flowers, the family asks that you make a donation in Jay's name to either the Lewy Body Dementia Association or the Parkinson's Association so that one day no other family will have to watch a loved one experience these awful diseases.
