He was predeceased by: his father Charles D. Sullivan. He is survived by: his mother Helen M. Sullivan (Taylor) of Waltham; his close friend Deborah M. Nichols; and his siblings, Lynne Thambash of Woburn, Julie Sullivan of Waltham, Kathleen Green of Idaho and Charles "Chip" Sullivan Jr. of Waltham. He is also survived by many nieces and nephews.
In lieu of flowers expressions of sympathy may be made in his memory to the American Cancer Society or the American Heart Association.
