He is survived by: his wife Michele; his siblings, Mat Hazlett of Ekron, KY, Ken Clark, Mark Clark and Roseanne Ackerly; his nephews, Kevin Blanchard and Corey Couture of Turners Falls; and his niece Nicole Couture of Bernardston.
