He was predeceased by: his parents, Alex Levesque and Helen Benson (Cummings). He is survived by: his wife Ann Levesque (White); and his sister Sheila Levesque. He is also survived by many nieces and nephews.
In lieu of flowers, memorial gifts made to Dana Farber Cancer Institute in David's memory, would be deeply appreciated.
