He was predeceased by: his mother Jacqueline Humphries; his grandmother Annette Wilson; his great-grandparent Willie Humphries, Sr.; his grandparents, Addie Humphries, Mildred Humphries, Frank, Nadine Turpin (Byrd), Sarah Louise Williams and James Dexter Williams; his uncle Tyrone Flex Humphries; and his aunt Cassandra Mattas.
He is survived by: his wife Brittany Polk (Moorehead) of Springfield; his children, Jayden, Jaylen, Londen Polk, Jaida and Lauryn Polk of Springfield; his adoptive parents, Antonio Polk and Kathy Polk; his father Leslie Dexter Williams (Dean) of Springfield; his grandparents, Beverly Humphries and Willie Humphries Jr.; his siblings, Tyrone Polk of Springfield, Leon Williams of Boston, Jarred Williams of Lakeville, MN, Zayden Williams of Springfield, Michael Roman (Zenita) of Springfield, Daqonda Humphries of Worcester and Angelica Polk of Springfield; his stepsister Zahara Fletcher of Springfield; and his mother-in-law Dr. Monsita J Moorehead. He is also survived by uncles, aunts, nieces, nephews, cousins, church family, family friends.
