He was predeceased by: his parents, Ward R. Shiveley and Bonita V Shiveley (Jewett); his brothers, Robert, Ward and D. Michael; his sister-in-law Claire; and his nephew Robert.
He is survived by: his wife Annie Shiveley (Frechette) of Southwick, MA; his daughters, Mechelle Farnsworth (Jeff) of Lake Wales, FL and Cyndee Shiveley (Jay Kusnierz, Sr.) of Southwick, MA; his grandchildren, Morgan (Jeremiah), Jay Jr., Kara, Dustin and Tanner; his great grandchild Emma; his sisters-in-law, Mary Leary (James), Shirley Nelson and Patricia Martin; and his nieces and nephews, Brian, Todd, Scott, Kevin, Beth, Karen and Jodi. He is also survived by many other family members and friends.
