He is survived by: his parents, Joseph Kubic and Hazel Kubic (Henderson); his children, Sarah Kubic and Eric Kubic (Jennifer); his granddaughter Rylee; his former spouse Andrea Cook; his brother Michael Kubic (Cheryl) of Dallas, TX; his aunt Janice Mildram of Wells, ME; and his brother-in-law Russell Cisek (Susan).
