He was predeceased by: his parents, Stephen Michalak and Mary Michalak (Warlinska); his wife Jane Michalak (Smith); and his siblings, Joseph, Carl, William Michalak, Frances Milde, Rose McArdle, Vandis Jewell and Irene Hammarstrom.
He is survived by: his children, Frank Michalak II (Jeannine) of California and Lisa Michalak (Robert Bruzzese) of Worcester. He is also survived by many nieces and nephews.
