He was predeceased by: his parents, Anthony D'Agostino and Luisa D'Agostino (Delmonaco); his siblings, Philomena, Italo, Larry D'Agostino, Maurice, Salvatore, Peter Mastrapasqua, Bettina Larivee and Grace Thomas; and his sister-in-law Alice D'Agostino.
He is survived by: his wife Pina Coppellotti; his children, Donna D'Agostino (Larry Landry) of Worcester, Phillip D'Agostino (Lori) of Worcester, Joseph D'Agostino (Rosa) of Shrewsbury, Lisa Govoni (Andrew) of Shrewsbury and Linda Rothera (James) of Worcester; and his siblings-in-law, Phyllis D'Agostino, Nello and Orlando Coppellotti. He is also survived by many nieces, nephews, extended family members, and friends.
