He was predeceased by: his father Chester Cook, Jr.; his brothers, William Cook and James Cookke; and his grandparents, Chester, Ethel Cook, Leon and Mary Beauvais.
He is survived by: his mother Jean Cook (Beauvais) of Salem; his children, Brad Sullivan (Heather) of Groveland and Amanda Lemieux (Matthew) of Peabody; his grandchildren, Jackson Sullivan, Carly Sullivan, Josh Lemieux and Evan Lemieux; his siblings, Christina Espinola Lanzetta (Mark) and Michael Cook (Patti); his sister-in-law Jennifer Cookke; and his aunt Carol Cook (Roxbury). He is also survived by many nieces, nephews and friends.
