He was predeceased by: his parents, Kenneth W. Cook and Frances M. Cook (Corbin). He is survived by: his wife Sandra D. Cook (Shroyer); his children, Sheri L. Workman (David) of Westborough, Donna J. Goodell (Gregory) of Hudson, NH and David M. Cook (Amber) of Brentwood, CA; his sister Linda Pfeiffer of Lebanon, TN; and his grandchildren, Kendra, Sarah, Jacob, Adam, Ryan and Hayden.
