He is survived by: his father Frank Berkowitz; his wife Rachel (Bornstein); his sons, Jason and David; his grandchildren, Lexi, Zack, Chase and Brynn; and his brothers, Barry Berkowitz (Barbara) and Steve Berkowitz.
