He is survived by: his parent Bertha Cook (Johnnesen); his wife Anita Cook (Patterson); his sons, Thomas Cook of Florida and Barry Cook; his grandchildren, Thomas, Charles, Phillip and Sunshine; and his brothers, Russel Cook of Fairfax, VA and Robert Cook of Sparks, NV..
