He was predeceased by: his parents, Dimitrios Koutalakis and Irene Koutalakis. He is survived by: his wife Panagiota Koutalakis (Maliotakis); and his children, Charles Koutalakis, James Koutalakis (Rosa) and Irene Vulgaris (George). He is also survived by many relatives and friends.

In lieu of flowers, donations in his memory may be made to St. Catherine Greek Church.
