He was predeceased by: his parents, Vena Blake (Dewey) and Charles S. Blake; his wife Catherine L. Blake (Wade); his brother Charles S. Blake; and his sister-in-law Marian Blake (Hubbard).
He is survived by: his children, David K. Blake, Jr. (Marylou), Stephen A. Blake (Colleen) and Amy C. Blake-Baldwin (Jason); his grandchildren, Connor, Brianna, Elizabeth, Diana and Laura; his great grandchildren, Liv, Blakely and Austin; and his siblings, Richard D. (Jeanne M.) and Linda B. (Delma A. Smith). He is also survived by many nieces, nephews, great nieces, and great nephews.
