He is survived by: his mother Hilda Cusack (Reines) of South Hadley; his children, Patrick Cusack Jr. of North Carolina, Kimberly of Turners Falls and Charlene Waryasz of Greenfield; his sisters, Diana St (David) and Alice Patruno (Steve) of South Hadley; his grandchildren, Eric, Andrew and Piper; and his great grandchild Connor. He is also survived by nieces and nephews.
