He was predeceased by: his father Michael E. Kozlowski. He is survived by: his mother Christine J. Kozlowski (Russo) of Millbury; his significant other Michele Ferraiuolo; his brother Jeffrey A. Kozlowski (Stephanie); his niece Brooke; his nephew Nathan; and his pets, Liberty, Heidi, Buster, Poppy and Shadow. He is also survived by aunts, uncles and cousins.
