He was predeceased by: his parent John R. Evans; and his siblings, Constance, Cynthia and Richard Evans. He is survived by: his parent Virginia Evans (Gorman) of Framingham; his spouse Dianne Evans (Ulrich) of Milford; his daughter Colleen L. Chase (Christopher M. Chase) of Boylston; his grandson Derek M. Chase; and his siblings, James Evans (Meli) of Texas, Joseph Evans (Paula), William Evans (Deborah) of Amherst, Mary Evans of Framingham, Joan Keating of Virginia and Nancy Evans (Doug Kreb) of Maine.

In lieu of flowers, donations in Jack's name may be made to Lourdes Center.
