He was predeceased by: his parents, Donald E. Hazard and Hattie L. Hazard (Barr). He is survived by: his wife Frances E. Hazard (Varey); his children, Kimberly Latifi (Aziz) of Holliston, Shawn Hazard (Debra) and Amy Brayley (Michael) of North Attleboro; his brothers, Dr. Gerald Hazard (Anne) of Barnstable and Ronald Hazard (Patti) of Utah; and his grandchildren, Chelsea, Taylor, Hannah, Anissa, Zakaria, Alyssa, Chris and Tyler.
