He was predeceased by: his mother Patricia Costarelli (Sullivan) of Medford. He is survived by: his children, Brady Costarelli and Hazel Costarelli; the mother of his children Stephanie Czerurinski of Medford; his sister Lauren Farnham (Michael) of Melrose; and his father Steven Costarelli of Somerville. He is also survived by many loving aunts, uncles, cousins, nieces and nephews.
