He was predeceased by: his parents, James Blake and Mary Blake (Wright); and his sister Susan. He is survived by: his sister Karen DiSessa (Tony); his niece Kristen Oldakowski (Joseph); his nephews, Adam DiSessa and Mark DiSessa; and his cousin Judith Guthro.

Donations in his memory may be made to the Kaplan Family Hospice House 78 Liberty Street, Danvers, MA 01923.
