She was predeceased by: her sister Sally A. LeBlanc. She is survived by: her children, Brooke A. Parziale of Lynn and Andrew J. Gustin of Saugus; and her siblings, William (Laurie Forman) of Nashua, NH, Richard T. Forman of Lynn, Nancy Kimball of New Bern, NC and Beth Forman of Lynn. She is also survived by two grandchildren, several nieces and nephews.
Donations may be made in her name to the Salvation Army of Lynn Massachusetts.
