He was predeceased by: his parents, Thomas and Olive Costello Jr. (O'Brien); and his sister Kathie Jackman. He is survived by: his wife Joanne Costello (LaFave); his sons, Thomas, Daniel and David (Nicole); his siblings, Darby (Stephen Frank), Cris Costello and Brian Costello; and his grandchildren, Bella, Nolan and Philip Costello. He is also survived by many nieces and nephews and many many friends that were like family and family that were friends.
