He was predeceased by: his parents, Kenneth Cook and Barbara Ventura (Pierce); and his sister Marylouise Hurd. He is survived by: his sister Terri Avilla (Gus); his nieces and nephews, Shane, Timothy, Devon Avilla, Rebekah Carmichael and Megan Bassett; his uncle Jerry Pierce; and his aunt Helen Carmichael.
