He was predeceased by: his wife Mary A. Forde; his daughters, Doreen, Margaret and Angela Forde; his siblings, Maureen Wagner, Tony Forde and Patrick Forde; and his parents, Anthony Forde and Margaret Forde (Donoghue).
He is survived by: his sons, Philip C. Forde, Jr. (Rochelle) and Anthony P. Forde (Dorothy); his grandchildren, Amal Wing, Caitlin Joyal, Cassandra Favart, Philip C. Forde III, Samantha, Tiernan, Alexander and Joshua Forde; his great grandchildren, Olivia, Thomas, Michael Wing, Asher Rafique and Charlie Marks; and his siblings, Doreen McEvilly, Marguerita Rickard (Pat), Mary Conlon, Edward Forde (Ann) and Ann Forde.
