He was predeceased by: his parents, Bernard R. Hazelwood and Anne Hazelwood (Reardon); his brother Jeffrey Hazelwood; and his niece Karen Hazelwood. He is survived by: his siblings, Judyth Barrett of Holden and Ronald Hazelwood (Judy) of Shrewsbury; and his sister-in-law Venetia Hazelwood.
