He was predeceased by: his parents, Malcolm Knight Lewis and Mary Dargie Lewis (Fyfe); and his siblings, Marlynn P. Lewis, Priscilla Johnson (Lewis) and Wallace M. Lewis.
He is survived by: his children, D. Palmer Lewis, Jr., Alison L. Opolski (Joseph) of New Durham, N. H., Cabot J. Lewis (Kristin) of Andover and Jonathan F. Lewis (Matthew Boethin) of New York City; and his grandchildren, Stephanie Opolski, Kathryn Opolski, Emily Opolski, Benjamin Lewis, William Lewis, Caroline Lewis and Cabot Lewis, Jr.. He is also survived by several nieces and nephews.
In lieu of flowers, contributions can be made to High Pointe Hospice House, 360 North Avenue, Haverhill, MA 01830, or to Warde Rehab and Nursing, 21 Searles Road, Windham, NH 03087, both of which contributed greatly to Donald's comfort and care.
