He was predeceased by: his mother Karen M. Cloyd. He is survived by: his father David Church (Beth); his son Camden; his brothers, Josh D. Church of Haverhill and Adam M. Rizzotti of Haverhill; his grandparents, Frank M., Jr., Janet E. Cloyd (Bradley) of Haverhill and Susan E. Church of Jacksonville, FL; and his friend Marisa Moughan of West Newbury. He is also survived by many aunts, uncles, cousins.
