She was predeceased by: her parents, Walter Phillips and Mildred Phillips (Chase); her daughter Michelle; and her brother Walter C. Phillips. She is survived by: her son Dana Lively; and her sisters, Dr. Melinda Gougeon of Ashfield and Suzanne Fish of Greenfield. She is also survived by eight grandchildren.
