He is survived by: his parents, Eva Bolack (Benoit) and Francis Bolack; his siblings, Tracy Jolie (David), Diane Caputo (Lance) and David Bolack (Trish); his nieces and nephews, Crystal Wypyszinski, Madison, Olivia Jolie, Arielle, Jordana, Justina, Chelsea, Lauryn, Leonard, Davis and Clark Caputo; his great-nephew Robert; and his great-niece Amanda.
