He was predeceased by: his parents, Frank J. Poliks and Marion Poliks (Domkowski). He is survived by: his wife Felicia Poliks (Kulczyk); his children, Dr. Mark D. Poliks (Barbara Poliks) of Vestal, NY, Christine Poliks of Melrose, MA and Carolyn Poliks (Joseph MacIndewar) of Melrose, MA.; and his grandchildren, Dr. Marek Poliks (Katherine Lamp), Elizabeth Poliks, Ania MacIndewar and Lea MacIndewar.
