She was predeceased by: her husband John J. Polak, Sr.; and her parents, Armand Hetu and Clara Hetu (Deslauriers). She is survived by: her children, John J. Polak, Jr., Michael Polak (Maura) and Karen Donahue (Polak); her grandchildren, Nathaniel, Heidi, Caroline, Sophie and Phoebe; and her great grandchildren, Jeffrey and Vivian.
