Mondeau (Robert) of Bridgewater and Eric Jacobson (Melissa) of Easton; his grandchildren, Jessica Kehoe of Bridgewater, Cayce Constantine of Easton, Hailey Mae of Easton, Chelsea Rose of Easton and Ashley Grace Jacobson of Easton; his great grandchild Ellie Constantine of Easton; his brother Arthur Jacobson (Lorraine) of Port Orange, FL; and his siblings-in-law, David Bagge (Cathy), Nancy Sullivan (Bill) of Easton, MA and Judi Macord (Joe) of Campton, NH. He was also cherished by many nieces and nephews.
In lieu of flowers donations in Jake's memory can be made to Dana-Farber Cancer Center or Old Colony Hospice.
