She was predeceased by: her parents, Joseph Tkacik and Mary Tkacik (Androsko); and her husband Raymond W. Bernard. She is survived by: her daughters, Jane Bradshaw of North Smithfield, RI, Julie Bernard (Kevin Bridges) of Saco, ME and Jana Demers (Jeff) of Dudley; her brothers, Joseph Tkacik of Thompson, CT, Edward Tkacik of Southbridge and Dick Tkacik of Webster; and her caregivers, Tarah Traina, Jackie Duval, Jack and Rose Sadowsky. She is also survived by four grandchildren and three great-grandchildren.
