He was predeceased by: his parents, Anna Forman and Samuel Forman; his son Daniel Forman; and his siblings, Sid Forman, Ruth Witten, Max Forman, Sylvia Piken and Hy Forman.
He is survived by: his wife Mary "Connie" Forman; his son Richard Forman; his brother Harvey Forman of Peabody; and his grandchildren, William, Daniela Kai and Nikyta Rose Forman.
