He was predeceased by: his parents, Harry Kusiak and Anna Kusiak (Strycharz); his wife Martha Faith Kusiak (Harvey); and his brothers, Edward and Raymond Kusiak.
He is survived by: his children, John R. Kusiak (Chere) of Bradford, MA, Gail A. Boyd (Bill) of Chicopee and Jeffrey J. Kusiak of California; his grandchildren, Charles Kusiak, Nicole Demos, Merisa Boyd, Elliot Kusiak and Alexandra Kusiak; his great grandchildren, Madison and Michael; and his sister Harriet Supernant of Chicopee. He is also survived by nieces and nephews.
