He was predeceased by: his father Diamantino Sousa Costa. He is survived by: his mother Maria Do Carmo Costa (Dias) of Chicopee; his siblings, Maria Magnolia (Robert) of Chicopee, John Costa (Kim) of Palmer and Ricardo Costa of Chicopee; and his nieces and nephews, Amy Desautels, Michael Zabowski, Nikita White, Natasha and Amanda Costa.
Due to "Freddys" love of children and past involvement with the Shriners, the family asks that, in lieu of flowers donations be made to the Shriners Hospitals for Children, 516 Carew St, Springfield, MA 01104.
