He was predeceased by: his parents, Lawrence Bazan and Julia Bazan (Minkalis); and his brother Kenneth Bazan. He is survived by: his sister Jean Sullivan (James) of Chicopee; his niece Kim Murphy (Dave) of South Hadley; and his nephews, Kevin Sullivan of Chicopee and Keith Sullivan (Chris).

Memorial contributions may be made to the Soldiers' Home in Holyoke, 110 Cherry St., Holyoke, MA 01040.
