She was predeceased by: her husband John Cusick; her parents, Harry Smith and Gertrude Ingram Smith; and her brothers, Wilmer Smith (Bill) and Edward Smith.
She is survived by: her children, Brian Cusick (Susan) of Leominster and Jane E. Spellenberg of Chelmsford; her grandchildren, Amy Cusick (Dale Thomas), Colleen Cusick, Tim Cusick (Liza Webster), Molly Cusick, Bridget Koryak (Misha), Samantha Ellinwood (Scott) and John Spellenberg (Adam Musto); and her great grandchildren, Alexander, Avery, Maggie and Camden.
