He was predeceased by: his parents, Ernest Forni and Lillian Forni (Johnson); and his brother Ernest H. Forni Jr. (Gail) of West Yarmouth. He is survived by: his children, Joanne M. Sweeney (Edward), Kathryn J. King (Gilbert) of Wareham, Ralph M. Forni Jr. (Fatima) of Westport, Susan E. Gaspar (Charles) of Dartmouth and Melissa E. Villanueva (Joe Fernandes) of Dartmouth. He is also survived by many loving nieces, nephews, and dear friends.
