She was predeceased by: her parents, Eugene K. Baker and Alberta Baker (Brown); and her husband Ralph M. Forni. She is survived by: her children, Joanne M. Sweeney (Edward) of Florida, Kathryn J. King (Gilbert) of Wareham, Ralph M. Forni, Jr. (Fatima) of Westport, Susan E. Gaspar (Charles) of Dartmouth and Melissa E. Villanueva (Joe Fernandes) of Dartmouth; and her sister Phoebe Sherman of Wareham. She is also survived by several nieces and nephews.
