She was predeceased by: her husband Leonard "Lenny" Forman; her parents, Henry Bolton and Gertrude Bolton (Shoib); and her sister Ida Bolton. She is survived by: her daughters, Susan Stern (Warren) and Lynne Davis (Steven); her grandchildren, Jill Schwartz (Jonathan Titone), Peter Schwartz (Amy), Carly Speyer (Steve) and Aaron Davis (Meghan); and her great grandchildren, Benjamin, Alexander, Addison, Hunter and Benji.
