He was predeceased by: his wife Frances D'Agostino (Sweeney); his grandsons, Brian and Matthew Walsh; his brother Joseph D'Agostino; and his former wife Merna Lyon.

He is survived by: his children, Nancy Patierno (Frank) of Townsend, Ralph, Pam Martin of Woburn, Paul (Lisa) of Andover, Mary Walsh (Brian) of Woburn, Samuel (Beverly) of Medford, Grayce Flumerfelt (Carl) of Acton and Lisa Rose (Paul) of Burlington; his siblings, Norma Spano, Ralph D'Agostino and Robert D'Agostino; his sister-in-law Mary Buckley; and his stepchildren, JB, Amy and Cynthia. He is also survived by many loving nieces and nephews.
