He is survived by: his mother Mary Middlebrook of Burlington; his wife Elizabeth Stephens; his sons, Michael, Christopher and Eric; his daughters-in-law, Laura and Daniela Stephens; his siblings, John Stephens, Sandra Murphy, Randy Middlebrook and Denise Matthews; and his grandchildren, Mikayla Laverdure, Brooklyn Boisvert, Madeline Stephens, Lucas, Jordan and Dylan Stephens. He is also survived by his family and friends.
